Following primary lateral patellofemoral ligament reconstruction (LPFLR), the patient presented with recurrent superior and medial patellar instability after failure of her surgical construct during rehabilitation. A novel revision procedure was performed using a double-limbed semitendinosus allograft for LPFLR and iliotibial band autograft to reinforce the lateral patellotibial ligament. CONCLUSION: This report highlights the unique patient presentation of recurrent IMPI, a novel revision surgical technique, and biomechanical considerations associated with revision lateral patellofemoral complex reconstruction, adding to the limited literature on managing persistent IMPI.
