  • The previous NICE guideline recommended 'patient-controlled analgesia (PCA) using opioid analgesics should be offered after caesarean birth (CB) because it improves pain relief.' However, this recommendation was withdrawn in August 2019 because of safety concerns, particularly regarding the use of patient-controlled opioids in women who have received intrathecal opioids, and changes in practice in the UK. These changes include greater use of neuraxial opioids, widespread use of transverse (rather than midline) incisions which are associated with less pain and use of local anaesthetic blocks in the transverse abdominus plane in those requiring a general anaesthetic. There is also a reluctance to restrict women's mobility and ability to look after her baby with the use of an intravenous PCA. A number of women will obtain adequate analgesia with non-opioid medicines following caesarean birth, and the aim of this review is to identify the role of opioids in pain management following caesarean birth.
